How to Book Your Appointment

    Open the Scheduling Link

    Click the appointment link in your email or text message. You'll see our available times.

    Select Your Preferred Time

    Browse the calendar and choose a date and time that works for you.

  3. 3

    Confirm Your Details

    Enter your contact information. If you've worked with us before, your details may pre-fill automatically.

  4. 4

    Receive Your Confirmation

    You'll get a calendar invitation and automatic reminders before your appointment.

Need to Reschedule?

If your plans change, you can easily reschedule your appointment:

  • Click the reschedule link in your confirmation email
  • Or call us at 407-500-7000
  • You'll receive an updated calendar invitation with your new time

Please give us 24 hours notice

If you need to cancel or reschedule, please let us know at least 24 hours before your scheduled time.

Completing Forms

How Form Submission Works

    Access Your Form

    Click the link in your email, text message, or client portal. The form will open in your browser.

  2. 2

    Fill In Your Information

    Complete each section. If you've worked with us before, some fields may already be filled in for you.

  3. 3

    Submit When Complete

    Click the submit button. We'll receive your form immediately and store a copy in your file.

Request a Copy

After submitting, ask us for a copy of your completed form for your records.
